Westlake Nursing and Rehabilitation Center in Florida is seeking a compassionate Licensed Practical Nurse (LPN) to join our nursing team. The role focuses on delivering quality care for residents in a skilled nursing setting, with emphasis on collaboration and resident well-being.

Qualifications include an active Florida LPN license, graduation from an accredited PN program, CPR/BLS certification, and at least one year in long-term care. EMR experience and strong teamwork are required.

Responsabilités

• Administer medications, treatments, and therapies as prescribed.

• Supervise and support CNAs.
• Maintain accurate and timely documentation in the EMR.
• Assist with admissions, transfers, and discharges.
• Collaborate with physicians, therapists, social services, and other interdisciplinary team members.
• Ensure compliance with CMS, state, OSHA, HIPAA, and facility policies.
• Promote resident dignity, safety, and quality of life.
• Respond promptly to resident needs and emergency situations.
• Participate in care planning meetings, in-service education, and quality improvement initiatives.

Location: Westlake Nursing and Rehabilitation Center
Position: Licensed Practical Nurse (LPN)
Shift: 7:00 AM – 3:00 PM | 3:00 PM – 11:00 PM | 11:00 PM – 7:00 AM (Weekend and holiday availability may be required)

Duties and Responsibilities

• Administer medications, treatments, and therapies as prescribed.

• Supervise and support Certified Nursing Assistants (CNAs).

• Maintain accurate and timely documentation in the electronic medical record (EMR).

• Assist with admissions, transfers, and discharges.

• Collaborate with physicians, therapists, social services, and other interdisciplinary team members.

• Ensure compliance with CMS, state, OSHA, HIPAA, and facility policies.

• Promote resident dignity, safety, and quality of life.

• Respond promptly to resident needs and emergency situations.

• Participate in care planning meetings, in-service education, and quality improvement initiatives.

Qualifications

• Current, active Licensed Practical Nurse (LPN) license in the State of Florida.

• Graduate of an accredited Practical Nursing program.

• Current CPR/BLS certification.

• Minimum of One-Year Long-term care or skilled nursing experience required.

• Strong communication, organizational, and critical thinking skills.

• Ability to work independently and as part of a multidisciplinary team.

• Experience with electronic medical records (EMR) is required.
