Westlake Nursing and Rehabilitation Center is seeking compassionate, dependable LPNs to join our skilled nursing team in Florida. You will provide essential nursing care to residents, documenting in the EMR and collaborating with an interdisciplinary team.

Responsibilities include medication administration, care planning participation, and ensuring safety and quality of life. A Florida LPN license, CPR/BLS, and 1+ year LTC experience are required; shifts include weekends/holidays as needed.

Qualifications

• Current, active LPN license in Florida.
• Graduate of an accredited Practical Nursing program.
• Current CPR/BLS certification.
• Minimum of One-Year Long-term care or skilled nursing experience.
• Strong communication, organizational, and critical thinking skills.
• Ability to work independently and as part of a multidisciplinary team.
• Experience with electronic medical records (EMR) is required.

Responsibilities

• Administer medications, treatments, and therapies as prescribed.
• Monitor and assess residents, reporting changes in condition to the RN and physician.
• Supervise and support CNAs.
• Maintain accurate and timely documentation in the EMR.
• Assist with admissions, transfers, and discharges.
• Collaborate with physicians, therapists, social services, and other interdisciplinary team members.
• Ensure CMS, state, OSHA, HIPAA compliance and facility policies.
• Promote resident dignity, safety, and quality of life.
• Respond promptly to resident needs and emergencies.
• Participate in care planning meetings, in-service education, and quality improvement initiatives.

Shift: 7:00 AM – 3:00 PM | 3:00 PM – 11:00 PM | 11:00 PM – 7:00 AM (Weekend and holiday availability may be required)
