Holton Manor in Elkhorn, WI is seeking a Licensed Practical Nurse (LPN) to provide clinically competent care to residents under supervision and in accordance with established policies. You will interact with residents, families, and health team members, following care plans and physician orders.

Qualifications

• Active LPN license in Wisconsin or licensed in any U.S. state eligible for reciprocity.
• CPR certification.
• Ability to meet physical and job-related requirements.
• English literacy with excellent written and verbal communication skills.

Responsibilities

• Provide safe, effective nursing care per care plans and physician orders.
• Administer medications and treatments accurately.
• Assess residents and report changes promptly.
• Document care in the EMR.
• Monitor performance and mentor CNAs.
• Ensure infection control and safety compliance.
• Completes all other duties as assigned.

Job Summary:

TheLPNmust beaclinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsibleparty, and all health team members.

Qualifications:

Required:

Preferred:

• Long-term care or skilled nursing experience
