Job summary

Holton Manor in Elkhorn, WI is seeking a Licensed Practical Nurse (LPN) to provide clinical care under supervision of a registered nurse. You will administer medications, monitor residents, and document care in EMR.

This on-site role offers flexible scheduling (Days, PMs, NOCs, weekends) with comprehensive benefits including health/dental/vision insurance, 401(k) with employer match, tuition reimbursement and EAP. English literacy and strong communication are required.

Qualifications

• Active LPN license in Wisconsin or eligible for Wisconsin licensure by reciprocity
• CPR certification is required
• Ability to meet physical and job-related requirements
• Excellent written and verbal communication skills in English

Responsibilities

• Provide safe, effective nursing care per care plans and physician orders
• Administer medications and treatments accurately
• Assess residents and report changes promptly
• Document care in the EMR
• Monitor residents and mentor CNAs
• Ensure infection control and safety compliance
• Complete all other duties as assigned

Job Summary:

TheLPNmust beaclinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsibleparty, and all health team members.

Required:

• ActiveLPN licenseinstate of Wisconsin orlicensedin any U.S state and be eligible for Wisconsin licensure by reciprocity.(in good standing)

Preferred:

• Long-term care or skilled nursing experience
• Clinical experience
