Qualifications

• Must have Bachelor's Degree in Physical Therapy and be a Licensed Physical Therapist (PT) in Illinois.
• Ability to perform patient transfers with assistance and maintain safety.
• Educate patients, families, and care teams using clear teaching methods.
• Evaluate clinical findings to determine PT diagnoses and outcomes.
• Document in accordance with CTSCP67 and regulatory standards.
• Supervise PTA and rehab technicians per regulations.
• Communicate patient status, discharge needs, and equipment requirements.
