Responsibilities

• Evaluates and treats patients of all ages with plans of care that incorporate goals, treatment, outcomes, and discharge planning.
• Demonstrates safe body mechanics and transfers with assistance as needed.
• Educates patients and families using effective teaching methods.
• Documents care and outcomes in the EMR.
• Communicates with referring providers about progress and recommendations.
