Job summary

FutureCare is seeking a licensed Physical Therapist to evaluate, diagnose and treat movement dysfunction for residents in Maryland. You will create plans to restore mobility and independence, collaborating with nursing staff and families to maximize functional outcomes.

Qualifications

• Bachelor of Science degree in Physical Therapy.
• Current license or eligibility for licensure in Maryland.
• Certification and demonstrated competency in identified areas of specialization.
• Ability to assess resident needs and develop and implement a comprehensive plan of care.
• Knowledge of accreditation standards and compliance requirements.

Responsibilities

• Review resident information and evaluate its impact on assessment and treatment.
• Conduct and document a thorough functional assessment of the resident.
• Develop and implement an individualized treatment plan with achievable goals.
• Comply with evaluation, treatment and documentation guidelines.
• Complete re-evaluations and supervise rehab staff as appropriate.
• Document treatment in the resident's medical record after each session.
• Revise treatment plans as needed throughout the course of care.
• Provide discharge summaries and education on procedures to residents and families.
• Work with nursing staff and families to maximize functional ability.
• Instruct residents and caregivers on ongoing exercise programs.
• Assist in referrals for additional services after discharge.
