Job summary

Linley Park Post Acute in Anderson, SC, is seeking a licensed Physical Therapist to provide high-quality, patient-centered therapy. You will evaluate residents, develop treatment plans, collaborate with an interdisciplinary team, and supervise therapy staff to support recovery and mobility goals.

This full-time role offers competitive pay of $40-$50 per hour plus benefits including health, dental, vision, life insurance, PTO for your birthday, and a 401(k) with company match.

Qualifications

• Degree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred).
• Active South Carolina PT license in good standing.
• Experience in skilled nursing or rehab setting preferred.
• Familiarity with PointClickCare and Casamba preferred.
• Strong clinical, leadership, and documentation skills.

Responsibilities

• Evaluate residents within required timeframes and develop individualized treatment plans.
• Provide therapy services in accordance with physician orders and care goals.
• Document evaluations, treatments, and progress notes per regulatory standards.
• Supervise PTAs, aides, and students in compliance with state practice act.
• Collaborate with interdisciplinary team members on resident care plans.
• Participate in care conferences, utilization review, and rehab meetings.
• Educate residents, families, and caregivers on treatment plans and discharge needs.
• Assist with durable medical equipment (DME) recommendations.
• Provide staff training and in-service education.
• Maintain compliance with all state, federal, and payer regulations.
