Job summary

Linley Park Post Acute in Anderson, SC is seeking a licensed Occupational Therapist (OTR) to provide high-quality, patient-centered therapy services. The role includes evaluation, treatment planning, team collaboration, and supervision of therapy staff, all aimed at improving residents' independence and quality of life.

You will evaluate residents, develop individualized treatment plans, document progress, and ensure compliance with state and payer requirements.

Qualifications

• Active South Carolina OTR license in good standing.
• Bachelor's in Occupational Therapy; Master/Doctorate preferred.
• Experience in skilled nursing or rehab setting preferred.
• Familiarity with PointClickCare and Casamba preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Deliver therapy per physician orders and goals.
• Document evaluations and progress per regulations.
• Supervise COTAs, aides, and students.
• Collaborate with interdisciplinary care team.
• Participate in care conferences and rehab meetings.
• Educate residents and families on therapy and discharge plans.
• Assist with DME recommendations.
• Provide staff training and in-services.
• Ensure compliance with state, federal, and payer requirements.
