Job summary

Empower Home Health - a LiveWell Partners Network Agency is seeking a Physical Therapy Assistant (PTA) to provide physical therapy services under the supervision of a licensed physical therapist. The PTA collaborates with the patient’s care team across internal disciplines and physicians to support the patient’s rehabilitation goals.

Responsibilities include assisting the PT with care plans, preparing notes, performing PT services per orders, and educating patients and families on home programs.

• Instruct patients and caregivers on in-home programs and activities of daily living.
• Participate in interdisciplinary case conferences to contribute to patient updates, discharge planning, etc.
• Advise and consult with agency personnel and patient family/caregivers.

Company:

Empower Home Health - a LiveWell Partners Network Agency

Compensation:

Pay per visit, $44 - $48 per routine visit, Minimum Expectation of 30 visits/week (60 visits bi-weekly), Mileage reimbursed at $0.67/mile, Payroll is processed bi-weekly (26 pay periods).

Shift Type:

Full-time, Monday - Friday, 8 AM to 4 PM (flexible), In the field/On the road at 15-30% of the average 40 hour work week.

Summary:

The Physical Therapy Assistant (PTA) provides physical therapy services to patients under the supervision of a licensed physical therapist.The Physical Therapy Assistant (PTA) works as a collaborative member of a patient’s care team, working across the internal interdisciplinary care team, physicians, and other stakeholders in our patient’s care.

• The Physical Therapy Assistant (PTA) confirms on a weekly basis the scheduling of visits with the supervising physical therapist, care team, office staff, etc.
