Job summary

Apollo Medical Home Health - a LiveWell Partners Network Agency seeks a Physical Therapist Assistant to support licensed therapists in patient care within home health settings. The PTA collaborates with the care team and follows therapy plans while educating patients and families for home programs.

Responsibilities

• Assist the physical therapist in following physical therapy care plans.
• Prepares clinical and progress notes and helps develop the treatment plan (revising as necessary).
• Performing PT services in line with the care plan and physician orders.
• Consult with the physical therapist regarding changes in treatment.
• Instruct patients and caregivers on in-home programs and activities of daily living.
• Participate in interdisciplinary case conferences to contribute to patient updates, discharge planning, etc.
• Advises and consults with agency personnel and patient family / caregivers.
• Confirms on a weekly basis the scheduling of visits with the supervising physical therapist, care team, office staff, etc.

Compensation:

Pay per visit, Per visit value between $38.00 and $49.50, Earning Potential of $2,700 - $2,940+ bi-weekly, processed bi-weekly (26 pay periods), Mileage reimbursed at $0.67/mil e.

Shift Type:

Full-time, Monday - Friday, 8 AM to 4 PM (flexible), In the field/On the road at 15-30% of the average 40 hour work week.

Summary:

The Physical Therapy Assistant(“PTA”) provides physical therapy services to patients under the supervision of a licensed physical therapist. The PTA works as a collaborative member of a patient’s care team, working across the internal interdisciplinary care team, physicians, and other stakeholders in our patient’s care.
