Job summary

Wyomissing Health & Rehabilitation Center in Reading, PA seeks a Physical Therapist Assistant (PTA) for full-time or flexible PRN positions. Join our in-house rehabilitation team and help residents regain mobility with a collaborative care approach.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Pennsylvania PTA license or eligibility for licensure.
• Indirect License accepted.
• SNF/LTC experience preferred but not required.
• New graduates are encouraged to apply.

Responsibilities

• Provide skilled physical therapy treatments under the supervision of a licensed Physical Therapist.
• Implement individualized treatment plans for short-term rehab and long-term care residents.
• Help residents improve mobility, strength, balance, endurance, gait, and independence.
• Monitor resident progress and communicate changes to the supervising PT.
• Educate residents and caregivers on exercises, mobility techniques, and fall prevention.
• Complete accurate clinical documentation using Net Health.
• Collaborate with nursing and therapy teams to support resident outcomes.
• Maintain compliance with federal, state, and facility regulations.

What You'll Do

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Implement individualized treatment plans for short-term rehabilitation and long-term care residents.
• Help residents improve mobility, strength, balance, endurance, gait, and functional independence.
• Monitor resident progress and communicate changes to the supervising Physical Therapist.
• Educate residents and caregivers on therapeutic exercises, safe mobility techniques, and fall prevention.
• Complete accurate and timely clinical documentation using Net Health.
• Collaborate with nursing, therapy, and other interdisciplinary team members to support positive resident outcomes.
• Maintain compliance with applicable federal, state, and facility regulations.

What We’re Looking For

• Graduate of an accredited Physical Therapist Assistant program.
• Current Pennsylvania PTA license or eligibility for licensure.
• Indirect License accepted.
• Previous Skilled Nursing Facility (SNF) or Long-Term Care (LTC) experience preferred but not required.
• New graduates are encouraged to apply!
• Familiarity with Net Health and PointClickCare (PCC) is a plus.
• Strong communication, organizational, and teamwork skills.
• A positive attitude and genuine passion for helping residents achieve their rehabilitation goals.
