Job summary

Wyomissing Health & Rehabilitation Center in Reading, PA is seeking a PRN Physical Therapist Assistant to join our in-house rehabilitation team. You will provide skilled PT support under the direction of a licensed PT, focusing on mobility, balance, and independence for long-term care residents.

We welcome graduates and those with an indirect license. Net Health experience is a bonus; flexible PRN hours and a collaborative team environment are offered.

Qualifications

• Current Pennsylvania Physical Therapist Assistant license or Indirect License eligible.
• Graduate of an accredited Physical Therapist Assistant program.
• SNF/Long-Term Care experience preferred; new graduates are encouraged to apply.
• Experience with Net Health is a plus.
• Strong communication, organization, and teamwork skills.
• Commitment to providing compassionate, resident-centered care.

Responsibilities

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Implement individualized treatment plans for short-term rehabilitation and long-term care residents.
• Assist residents in improving mobility, balance, strength, endurance, gait, and independence.
• Monitor resident progress and communicate changes to the supervising Physical Therapist.
• Educate residents and caregivers on exercises, mobility techniques, and fall prevention.
• Complete accurate and timely documentation using Net Health.
• Collaborate with nursing staff and interdisciplinary care team to optimize resident outcomes.
• Maintain compliance with federal/state regulations and facility policies.

Wyomissing Health & Rehabilitation Center is seeking a compassionate and motivated PRN Physical Therapist Assistant (PTA) to join our in-house rehabilitation team in Reading, Pennsylvania. This is an excellent opportunity for a PTA looking for flexible PRN hours while making a meaningful impact on the lives of residents in a Skilled Nursing and Rehabilitation setting.

As a member of our interdisciplinary therapy team, you'll help residents regain strength, mobility, and independence while delivering exceptional, resident-centered care.
