Job summary

Meadowview Rehabilitation & Nursing Center in Lafayette Hill, PA is seeking a compassionate Full-Time Physical Therapist Assistant (PTA) to join our in-house rehabilitation team. You will work under a licensed Physical Therapist to deliver evidence-based care, focusing on mobility, independence, and functional improvement for skilled nursing residents.

Qualifications include PA PTA license and graduation from an accredited PTA program.

Qualifications

  • Current PA PTA license.
  • Graduate of an accredited PTA program.
  • SNF/LTC experience preferred, new grads welcome.
  • Experience with Net Health and PCC a plus.
  • Strong communication and teamwork skills.

Responsibilities

  • Provide skilled PTA treatments under supervision of a licensed Physical Therapist.
  • Implement individualized rehab plans for short-term and long-term residents.
  • Help residents improve strength, balance, mobility and independence.
  • Monitor progress and communicate with supervising PT.
  • Educate residents and families on exercises and mobility techniques.
  • Document accurately using Net Health and PCC.
  • Collaborate with physicians and care team to optimize outcomes.
  • Comply with federal, state, and facility regulations.

Meadowview Rehabilitation & Nursing Center is seeking a compassionate and motivated Full-Time Physical Therapist Assistant (PTA) to join our dynamic in-house rehabilitation team in Lafayette Hill, Pennsylvania. This is an exciting opportunity to work alongside experienced therapists in a collaborative Skilled Nursing and Rehabilitation environment dedicated to providing exceptional patient care.

As a PTA, you'll play an essential role in helping residents improve mobility, restore function, and maximize independence while delivering personalized, evidence-based care.

Responsibilities
  • Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
  • Implement individualized treatment plans for short-term rehabilitation and long-term care residents.
  • Assist patients in improving strength, balance, mobility, endurance, gait, and functional independence.
  • Monitor resident progress and communicate updates to the supervising Physical Therapist.
  • Educate residents and families on therapeutic exercises, mobility techniques, and fall prevention strategies.
  • Complete accurate and timely documentation using Net Health and PointClickCare (PCC).
  • Collaborate with physicians, nursing staff, and the interdisciplinary care team to achieve the best possible patient outcomes.
  • Maintain compliance with all federal, state, and facility regulations.
Qualifications
  • Current Physical Therapist Assistant (PTA) license in the Commonwealth of Pennsylvania.
  • Graduate of an accredited Physical Therapist Assistant program.
  • Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but new graduates are encouraged to apply.
  • Experience with Net Health and PointClickCare (PCC) is a plus.
  • Excellent communication, organizational, and teamwork skills.
  • Passion for delivering high-quality, resident-centered care.
