Job summary

York Nursing & Rehab Center in Philadelphia, PA is seeking a compassionate Physical Therapist Assistant (PTA) to join our in-house rehabilitation team on a PRN basis. The role supports residents to achieve mobility and independence through personalized plans under licensed PT supervision.

Ideal candidates hold PA PTA license, have graduated from accredited PTA program, and can work independently within an interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Pennsylvania PTA license required.
• Indirect License Preferred.
• SNF/LTC experience preferred.

Responsibilities

• Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Implement individualized treatment plans designed to improve mobility, strength, balance, and functional independence.
• Monitor resident response to treatment and communicate progress with the supervising therapist and interdisciplinary team.
• Accurately complete treatment documentation using Net Health.
• Maintain compliance with company policies and state and federal regulations.
• Support a positive, resident-focused therapy environment.
