Job summary

Wyomissing Health & Rehabilitation Center in Reading, PA seeks a Part-Time or PRN Physical Therapist to join our in-house rehabilitation team. The role supports residents’ independence and functional goals in a SNF/LTC setting.

We welcome new graduates and experienced therapists seeking flexible hours with mentorship and a supportive leadership team. Net Health and PCC platforms are used for documentation and care coordination.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current PA PT license or license eligibility.
• Ability to work effectively in a SNF/LTC environment.
• Excellent communication and interpersonal skills.
• New graduates welcome to apply.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services in a SNF/LTC setting.
• Collaborate with nursing, therapy, physicians and the interdisciplinary team.
• Monitor resident progress and adjust treatment plans as needed.
• Complete timely and accurate documentation using Net Health.
• Promote resident mobility, safety, independence and quality of life.
