Job summary

Phoenix of Maple Heights in Maple Heights, OH is seeking a PRN Physical Therapist Assistant (PTA) to join our in-house rehabilitation team and support our skilled nursing residents. As part of the rehab team, you’ll provide treatments, work with supervising therapists, document progress, and strive to improve residents’ mobility and independence in a compassionate, resident-centered environment.

Ohio PTA license and graduation from an accredited PTA program are required; SNF/LTC experience and

• Must hold an active Ohio PTA license.
• Graduate of an accredited PTA program.
• SNF/LTC experience is a plus.
• Experience with Net Health is a plus.

Responsibilities

• Provide physical therapy treatments according to the established plan of care.
• Work closely with the supervising Physical Therapist.
• Help residents achieve functional mobility goals.
• Accurately document treatments and resident progress.
• Communicate with rehab and interdisciplinary teams.
• Provide compassionate, resident-centered care.
