Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Reading, MA. The role focuses on planning, organizing, developing, and directing occupational therapy services to ensure high-quality resident care under applicable standards and administrator direction.

Responsibilities include screening residents, implementing treatment plans, coordinating schedules with the team, documenting care, and ensuring CMS compliance.

Qualifications

• Must have active OTA licensure in state(s) of practice (MA)
• Ongoing continuing education to maintain licensure
• Ability to work in a skilled nursing facility setting

Responsibilities

• Screen residents to determine best intervention or treatment
• Develop and implement treatment plans under supervision
• Interpret and communicate evaluation findings to residents and families
• Schedule resident therapy with team members
• Document and bill occupational therapy services per plan
• Perform all duties to effectively treat patients
• Comply with CMS documentation standards and daily charting
