Job summary

Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant for an in-house skilled nursing facility in Reading, MA. The role focuses on planning, organizing, and directing OT services to ensure high-quality resident care in compliance with standards and regulations.

Responsibilities include screening residents, implementing treatment plans, communicating findings, scheduling therapy, documenting and billing, and maintaining CMS compliance.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Screen regular residents to determine the best intervention or treatment for each patient.
• Perform treatment plans and follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules with the team.
• Conduct therapy and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties to effectively treat patients as assigned.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.
