Evolve Therapy Services seeks a Certified Occupational Therapy Assistant to provide in-house therapy services at a Skilled Nursing Facility in Reading, MA. Qualifications

• Associate's degree in Occupational Therapy (OT)
• Must have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Conduct regular screenings to determine best intervention for residents.
• Perform treatment plans and monitor progress per plan of care under supervision.
• Communicate evaluation findings, plans, and recommendations to residents, families, and team members.
• Establish therapy schedules in coordination with the team.
• Document therapy sessions and ensure billing and CMS compliance.
