Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Reading, MA. The role is listed as Full Time or PRN, enabling flexible commitment based on facility needs.

The OTA will plan, organize, direct Occupational Therapy services, ensure quality resident care, and work under an administrator's direction to meet federal, state, and CMS standards. Active licensure and ongoing professional development are required.

Qualifications

• Associate's degree in Occupational Therapy.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment for each patient.
• Perform treatment plans to residents; follow short- and long-term goals as per plan of care by supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents and families/caregivers.
• Establish individual resident therapy schedules with team members.
• Ensure therapy is delivered per treatment plan and proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.
