Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility partner located in Reading, MA. This full-time position supports comprehensive occupational therapy services within a facility setting.

The role requires an Associate's degree in Occupational Therapy and an active OTA license in the state(s) of practice, with ongoing continuing education to maintain licensure and certification.

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment plan.
• Perform treatment plans aligned with goals and plan of care.
• Communicate evaluation findings and recommendations to residents, families, and team members.
• Establish resident therapy schedules with the team.
• Document and bill for occupational therapy services per plan.
• Comply with CMS documentation standards daily with point-of-care documentation.
