Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Reading, MA. The role focuses on planning, organizing, and directing occupational therapy services in the facility to ensure high-quality resident care under the Administrator's direction.

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine interventions based on scope of practice.
• Perform treatment plans following established goals and plan of care.
• Interpret and communicate evaluation findings to residents, families, and team.
• Establish individual resident therapy schedules with team coordination.
• Conduct therapy according to plan and ensure documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily.
