Qualifications

• RN licensed with current practice status.
• Experience in Long Term Care preferred.
• English language proficiency required.

Responsibilities

• Assesses health status and level of care for new admissions.
• Ensures accurate completion of MDS and PPS documentation.
• Oversees data entry and electronic submission of MDS assessments.
• Coordinates interdisciplinary participation for MDS across all shifts.
• Maintains scheduling for MDS assessments with regulatory dates.
• Communicates care levels to all disciplines.
• Assists in formulating and revising resident care plans.
• Performs quality checks and ensures compliance with state/federal standards.
• Conducts resident care conferences and ensures MDS reviews are complete.
