Job summary

iCare Health Network is seeking an MDS Coordinator to manage the MDS process, documentation, and care assessments to support Medicare/Medicaid reimbursement. You'll collaborate with Nursing, Dietary, Social Services, and Rehabilitation teams and coordinate care conferences.

The ideal candidate holds an active MA RN or LPN license, has QA/improvement experience, and previous supervisory exposure in long-term care. This is a salaried, on-site role with a competitive compensation package.

Qualifications

• Active, unobstructed nursing license (RN or LPN) in MA required.
• Knowledge of Quality Assurance/Improvement processes, required.
• Experience in a supervisory position in long-term care, preferred.

Responsibilities

• Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process.
• Coordinates and facilitates care conferences with the interdisciplinary team.
• Coordinates, completes and reviews Minimum Data Sets (MDS).
• Educates, trains and assists nursing staff and other departments regarding the roles of assessments, planning, implementation and MDS process; and completion of applicable sections of MDS.
• Responsible for ensuring all current MDS/PPS updates are implemented and staff is in-serviced.
• Will cross train between PPS and OBRA.
• Conducts regular resident interviews as required by the MDS.
• Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed
