Job summary

Rock County is seeking a specialist to provide comprehensive, person-centered, team-based ACT to adults with severe mental illness through the Community Support Program. You will deliver case management, symptom management, supportive psychotherapy, psychosocial rehabilitation, and vocational supports aligned with individualized treatment plans.

Responsibilities include maintaining client records to meet DHS 63, Social Security and Medicaid/Medicare requirements, and collaborating with treatment

Qualifications

• Master's Degree in Social Work or related field required.
• 1500 CSP hours or 3000 hours with persons with severe and persistent mental illness.
• State of Wisconsin LCSW or LPC license required, or ability to obtain within 3 years.
• Must possess and maintain a valid unrestricted driver's license and auto insurance.
• Knowledge of DHS 63, Social Security and Medicaid/Medicare requirements.
• Ability to document and maintain accurate client records and reports.

Responsibilities

• Provide comprehensive, person-centered, team-based ACT to CSP clients.
• Develop and implement clinically relevant treatment plans aligned with recovery orientation.
• Assess consumer mental status and communicate findings to the team.
• Advocate for least restrictive living arrangements and testify in court as needed.
• Maintain client records, benefits, plans and reports per DHS 63 and funding requirements.
• Collaborate with treatment team, families and collateral agencies.
