Job summary

Human Touch Home Health in Virginia seeks a compassionate Licensed Master Social Worker (LMSW) to support patients in Richmond and surrounding communities. You'll perform psychosocial assessments, help navigate resources, and coordinate with physicians and the care team.

This PRN role offers flexible scheduling, independence, and meaningful impact on patients and families. A dedicated leader and diverse support network back you as you empower patients to maintain independence and improve quality

Qualifications

• Virginia LMSW license required.
• MSW from an accredited program.
• Valid driver's license and reliable transportation.
• Strong assessment, communication, and documentation skills.
• Home Health experience preferred but not required.
• Compassion for serving diverse patient populations.

Responsibilities

• Complete comprehensive psychosocial assessments.
• Identify emotional, social, environmental, and financial barriers to care.
• Connect patients and families with community resources and support services.
• Assist with discharge planning and long-term care planning.
• Provide counseling, education, crisis intervention, and emotional support.
• Collaborate with physicians and interdisciplinary team members.
• Advocate for patients while coordinating appropriate community services.
• Complete timely and accurate clinical documentation.
