Job summary

Human Touch Home Health is seeking a compassionate Speech-Language Pathologist (SLP) to join our team serving Richmond and surrounding communities. This PRN opportunity offers flexibility, professional independence, and the chance to make a life-changing impact in patients' homes.

As an SLP, you'll perform evaluations, develop treatment plans, provide home-based therapy, educate families, and document progress while collaborating with physicians.

Qualifications

• Active Virginia SLP license required.
• CCC-SLP certification preferred.
• Valid Driver's License and Reliable Transportation.
• Strong Clinical Assessment and Documentation Skills.
• Home Health Experience Preferred but Not Required.
• Experience with OASIS is a plus.

Responsibilities

• Complete speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on patient goals and clinical needs.
• Provide skilled speech therapy services in patients' homes.
• Educate patients and caregivers on communication techniques and swallowing strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians and interdisciplinary team members.
• Complete timely and accurate clinical documentation.
