Job summary

Human Touch Home Health in Richmond, VA is seeking a Licensed Master Social Worker (LMSW) to join our caring team. You will assess psychosocial needs, connect families with community resources, and support discharge and long-term care planning.

You will provide counseling, education, and emotional support while coordinating services with physicians and the interdisciplinary team. This role emphasizes compassionate, patient-centered care across diverse communities.

Qualifications

• Active Virginia LMSW license required.
• Master's Degree in Social Work from an accredited program.
• Valid driver’s license and reliable transportation.
• Home health or healthcare experience preferred but not required.
• Compassion for serving diverse patient populations.

Responsibilities

• Complete comprehensive psychosocial assessments.
• Identify emotional, social, environmental, and financial barriers to care.
• Connect patients and families with community resources and support services.
• Assist with discharge planning and long-term care planning.
• Provide counseling, education, crisis intervention, and emotional support.
• Collaborate with physicians and interdisciplinary team members.
• Advocate for patients while coordinating appropriate community services.
• Complete timely and accurate clinical documentation.
