Job summary

Human Touch Home Health in Fredericksburg, VA is seeking a licensed Master Social Worker (LMSW) for PRN home health opportunities across Fredericksburg and surrounding communities.

You will complete psychosocial assessments, identify barriers to care, connect families with community resources, assist with care planning and discharge planning, and provide short-term counseling to support patients remaining safely at home.

Qualifications

• Master's Degree in Social Work required.
• Virginia social work licensure required.
• Strong assessment, communication, and interpersonal skills.
• Knowledge of community resources and social service programs.
• Ability to work independently in patients' homes.
• Valid driver's license and reliable transportation.
• Home health or healthcare social work experience preferred.
• WellSky experience is a plus.

Responsibilities

• Complete psychosocial assessments in patients' homes.
• Identify social, emotional, financial, and environmental barriers affecting care.
• Connect patients and families with community resources and support services.
• Assist with long-term care planning and available financial resources.
• Provide short-term counseling and emotional support.
• Assist patients and caregivers with navigating community programs and services.
• Advocate for patient needs and help address barriers to remaining safely at home.
• Collaborate with nurses, therapists, physicians, and other members of the care team.
• Participate in care planning and discharge planning.
• Complete accurate and timely documentation using WellSky.

Patient opportunities may include Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, Orange, Locust Grove, Bowling Green, Colonial Beach, and nearby areas.

You do not have to cover the entire territory. Build your caseload around your availability and the areas that work best for you.
