Job summary

Human Touch Home Health is seeking a compassionate Speech-Language Pathologist (SLP) to join our team serving Richmond and surrounding communities. You will provide home-based speech therapy, evaluations, and customized treatment plans to improve patients' communication and swallow safety.

Licensed SLP in Virginia with CCC-SLP preferred, valid driver's license, and strong interpersonal skills are essential. Home health experience is a plus, with opportunities for growth and flexible scheduling.

Qualifications

• Active Virginia Speech-Language Pathologist license required.
• CCC-SLP certification preferred.
• Valid Driver's License and reliable transportation.
• Strong Clinical Assessment and Documentation Skills.
• Excellent Communication and Interpersonal Skills.
• Home health experience preferred but not required.
• Experience with OASIS is a plus.

Responsibilities

• Complete speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on patient goals and clinical needs.
• Provide skilled speech therapy services in patients’ homes.
• Educate patients and caregivers on communication techniques and swallowing strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians and interdisciplinary team members.
• Complete timely and accurate clinical documentation.

As a Home Health Speech-Language Pathologist, you’ll help patients overcome challenges related to speech, language, cognition, communication, and swallowing while improving their independence and quality of life in the comfort of their own homes.
