Job summary

Health Connect America is seeking a licensed clinician to provide Family Centered Treatment (FCT) services for children, adolescents, and families. The role focuses on direct in-home and community-based interventions with ongoing coordination to ensure fidelity to the FCT model.

Qualified candidates hold a Master’s degree in counseling or social work and must obtain FCT certification within a year. Ohio licensure (LSW/LISW/LPC/LMFT/LPCC) is required for this position.

Qualifications

• Master’s degree in Counseling, Social Work, or related field is required.
• Two years’ experience working with youth or families is required.
• Current Family Centered Treatment Certification is preferred.

Responsibilities

• Provide Family Centered Treatment (FCT) Clinical Services for children, adolescents, and families.
• Complete FCT Checkoffs and trainings within designated deadlines.
• Deliver FCT services primarily in home and community with a family focus.
• Provide 24/7 on-call coverage for a caseload of families and children.
• Develop and coordinate discharge plans and coordinate with Treatment Team.
