Job summary

Health Connect America is seeking a licensed Family Centered Treatment Therapist to serve children, adolescents, and families across Ohio. The role involves delivering FCT services primarily in-home and in the community, with on-call coverage to support families.

Minimum requirements include a Master’s degree in Counseling, Social Work, or related field, plus two years’ experience with youth or families. FCT Certification is preferred; Ohio licensure (LSW/LISW/LPC/LMFT/LPCC) is required.

Qualifications

• Minimum Master’s Degree in Counseling, Social Work, or related field.
• Two years’ experience working with youth or families.
• Current Family Centered Treatment Certification is preferred.

Responsibilities

• Provide Family Centered Treatment (FCT) Clinical Services as per the service definition and model fidelity requirements.
• Deliver FCT clinical services to children, adolescents, and their families in home and community settings.
• Provide 24/7 on-call coverage to a caseload of families and children.
• Complete Clinical Records and FCT documents per company standards and upload timely.
• Develop and coordinate discharge plans and coordinate with Treatment Team.
