Job summary

Health Connect America, Inc. is seeking a licensed Family Centered Treatment Therapist to deliver FCT services to children, adolescents, and families in their homes and communities.

The role requires ongoing certification, adherence to service plans, and collaboration with the Treatment Team to ensure quality care. The position emphasizes comprehensive documentation, on-call flexibility, and discharge planning with aftercare resources, as part of a person-centered care approach within our

Qualifications

• Master’s degree in Counseling, Social Work, or related field.
• Two years’ experience working with youth or families.
• Current Family Centered Treatment Certification is preferred.
• Must hold Ohio professional licenses: LSW, LISW, LPC, LMFT, or LPCC.

Responsibilities

• Provide FCT clinical services to children, adolescents, and families in home and community settings.
• Complete FCT checkoffs and internal trainings on schedule.
• Deliver psychoeducation per Service Plan.
• Develop and coordinate discharge plans with aftercare needs.
• Coordinate assessments and advise Treatment Team on service plans.
