Job summary

Health Connect America is seeking a Family Centered Treatment (FCT) Therapist to deliver intensive in-home clinical services for children, adolescents, and families within a structured, phase-based treatment model. You’ll coordinate with schools, medical providers, and child-welfare systems to ensure integrated care and durable improvements.

Requires a Master’s degree in Social Work, Counseling, or related field and two years’ experience with youth or families; current FCT certification is

Qualifications

• Master's degree in Social Work, Counseling, or related field required.
• Two years’ experience working with youth or families.
• Current Family Centered Treatment Certification is preferred.

Responsibilities

• Provide Family Centered Treatment (FCT) services in alignment with model fidelity requirements and clinical service definitions.
• Deliver in-home and community-based therapy to children, adolescents, and families using a structured, phase-based treatment model.
• Develop, implement, and adjust service plans in collaboration with treatment teams, families, and other involved systems.
• Facilitate family engagement, goal setting, psychoeducation, and skill-building aligned with identified treatment needs.
• Coordinate care with schools, medical providers, child welfare, juvenile justice, and natural supports to ensure integrated service delivery.
• Maintain accurate, timely documentation, complete all required FCT forms, and support discharge planning and aftercare coordination.
