Job summary

Desert Oasis Healthcare seeks a Senior SNF Care Management Manager to lead care coordination for Skilled Nursing Facility members and oversee SNF staff, ensuring quality patient care and timely transitions.

The role requires California RN license, BSN preferred, and at least 3 years of acute care experience; strong leadership and collaboration with directors to optimize metrics and workflows.

Qualifications

• RN license in California
• BSN is preferred
• Minimum 3 years of recent acute care nursing experience
• Ability to communicate effectively with multiple stakeholders in the organization
• Detail oriented with time-management and prioritization skills
• Proficient with Microsoft Office applications

Responsibilities

• Collaborate with Director of Care Management to ensure operations support continuity and quality patient care
• Provide leadership and oversight to assigned SNF staff and ensure adequate resources
• Monitor staff performance and facilitate coaching, training, and professional development
• Ensure timely and thorough performance evaluations in line with policy
• Track SNF Care Management metrics and present progress to management
• Use data to develop strategies to meet department goals
• Lead departmental meetings and resolve escalated staff/sNF issues
• Maintain relationships with contracted SNF facilities and providers to promote smooth patient transitions
• Ensure plans of care are reviewed for timely rehab progression
• Promote continuous process improvements across workflows and policies
