Job summary

Qualifications

Responsibilities

• Coordinate care along the patient care continuum and manage transitions.
• Lead and participate in multidisciplinary care conferences and complex case reviews.
• Assess patients at risk for readmission and plan post-acute care needs.
• Document thoroughly in the Tenet Case Management system and communicate with care teams.
• Ensure compliance with regulatory standards and Tenet policies.
