Job summary

Kintegra Health in Hickory, NC seeks a Behavioral Health Provider (LCSW or LCMHC) to join a multidisciplinary primary care team. You will screen, provide brief interventions, educate, and refer patients with psychosocial, behavioral health, chronic disease, and substance use concerns.

Under supervision of the Director of Behavioral Health, you will support primary care staff with treatment planning, ethical practice, and ongoing quality improvement.

Qualifications

• Master’s degree in a relevant field.
• Minimum two years' experience in a public health setting.
• Licensed Clinical Social Worker (LCSW) or Licensed Clinical Mental Health Counselor (LCMHC) in good standing with NC LSB.

Responsibilities

• Adheres to professional Code of Ethics while delivering behavioral health services.
• Provides evidence-based assessment, intervention, consultation, and referral for patients with behavioral and chronic health conditions.
• Works with the primary care team to improve patient care and team efficiency.
• Develops and implements behavioral health, substance use, and chronic disease management care plans.
• Assists primary care providers with screening, diagnosis, management, and treatment of behavioral health issues.
• Participates in ongoing assessment and quality improvement to support integration of primary care and behavioral health.
• Provides training and consultative support to health care team members as needed.
• Completes required documentation for patient care and billing in a timely manner.
• Maintains licensure through continuing education and liaises with community resources.
