Job summary

The position requires a Master’s degree, NC LCSW or equivalent, at least two years in public health and experience with frail elderly, plus competency in EMR documentation and collaboration with providers to improve outcomes.

Qualifications

• Master’s degree required in a relevant field.
• Minimum two years’ public health experience.
• At least 1 year with frail elderly population.
• Licensed Clinical Social Worker (or equivalent) in NC in good standing.
• CMS competencies training before independent work.
• Immunizations up to date and medical clearance for direct contact.

Responsibilities

• Maintain adherence to professional Code of Ethics.
• Provide behavioral and chronic health assessments, interventions, and referrals.
• Collaborate with an interdisciplinary healthcare team to improve patient care.
• Develop, implement, and monitor individualized treatment plans.
• Document encounters in the EMR and ensure timely clinical records.
• Participate in ongoing quality improvement efforts related to care integration.
• Provide consultative support and education to care team members.
• Maintain professionalism with participants, families, colleagues, and supervisors.
