Kintegra Health is seeking a Behavioral Health Provider (LCSW/LCMHC/LMFT) for full-time work in Lincolnton, NC. You will work within a multidisciplinary primary care team to deliver holistic behavioral health services, including screening, brief intervention, and referrals, under supervision of the Director of Behavioral Health.

Qualifications include a master’s degree and licensure (LCSW/LCMHC in good standing). A $2,500 sign-on bonus is offered for fully licensed providers.

Qualifikationen

• Master’s degree required for licensure.
• Licensed Clinical Social Worker (LCSW) or Licensed Clinical Mental Health Counselor (LCMHC) in good standing.

Aufgaben

• Adhere to professional Code of Ethics in practice.
• Provide evidence-based assessment, intervention, consultation, and referral services for behavioral and chronic health conditions.
• Collaborate with the primary care team to improve patient care and team efficiency.
• Develop and implement behavioral health, substance use, and chronic disease management plans.
• Assist primary care providers with screening, diagnosis, management, and treatment of behavioral health issues.
• Engage in ongoing assessment and quality improvement for behavioral health integration.
• Provide consultative services and training to health care professionals as needed.
• Complete required documentation for patient care and operations.
• Engage in continuing education to maintain licensure and competence.
• Maintain liaison with community resources and agencies.
