Job summary

Qualifications

• Licensure as LCPC, LCSW, LMFT or Licensed Clinical Psychologist per IDFPR.
• 2 years of behavioral health experience.

Responsibilities

• Performs diagnostic assessments and determines immediate treatment plan needs with patients.
• Completes treatment plans with patients identifying goals connected to assessment needs.
• Provides behavioral health therapy to individuals, families, and/or groups.
• Coordinates with Emergency Response Service and crisis teams for at-risk clients.
• Maintains records per UnityPlace policies and applicable regulatory bodies.
• Acts as liaison between the community and patient needs, coordinating with agencies and providers.
