Qualifications

• Ability to coordinate treatment plans and collaborate with care teams.

Responsibilities

• Performs diagnostic assessments and determines immediate treatment plan needs.
• Completes treatment plans with goals aligned to assessment findings.
• Provides behavioral health therapy to individuals, families and groups.
• Coordinates with crisis services for high-risk clients.
• Maintains records according to policy and regulatory requirements.
• Acts as liaison between the community and patient needs, coordinating with agencies and providers.
• Meets clinical productivity standards and documentation standards.
• May supervise clinicians and students and participate in trainings.
