Job summary

Carle Health is hiring a Licensed Behavioral Health Practitioner who will assess, intervene, and consult on behavioral health needs in individuals and groups. This role includes crisis evaluation and planning, and requires licensure per IDFPR and a combination of master’s degrees in related fields.

Responsibilities include diagnostic assessments, creating and following treatment plans, providing therapy, and coordinating with crisis services and community partners.

Qualifications

• Licensed clinicians with state licensure (LCPC/LCSW/LMFT/Clinical Psychologist) per IDFPR
• Master’s degrees in social work, psychology, counseling or related field
• Minimum 2 years of behavioral health experience

Responsibilities

• Performs diagnostic assessments and determines immediate treatment plan needs with patients.
• Completes treatment plans with patient goals/objectives.
• Provides behavioral health therapy to individuals, families and/or groups.
• Coordinates with emergency and crisis services for high-risk clients.
• Maintains records per organizational and regulatory standards.
• Coordinates treatment goals with community agencies, primary care physicians, court system and managed-care entities.
• Meets clinical productivity standards and documentation requirements.
• May supervise clinicians and students; participate in training and reporting.
