Job summary

Carle Health is seeking a Licensed Behavioral Health Professional to provide assessment, intervention, and consultation for individuals and groups with behavioral health needs. The role requires crisis evaluation when needed and adherence to state licensing rules.

Responsibilities include diagnostic assessments, treatment planning, and therapy for individuals, families, or groups, with coordination for emergency services and community partners.

Qualifications

• Licensed professionals: LCPC, LCSW, LMFT or Licensed Clinical Psychologist per IDFPR.
• 2 years behavioral health experience.
• Crisis evaluation and coordination with Emergency Response Service.

Responsibilities

• Performs diagnostic assessments, collects diagnostic information, and determines immediate treatment plan needs with patients.
• Completes treatment plan with patient, identifying goals/objectives that connect to needs identified in the assessment.
• Provides behavioral health therapy to individuals, families and/or groups with behavioral health needs.
• Coordinates with Emergency Response Service and other crisis service teams for clients who may be a danger to self or others.
• Maintains records per UnityPlace policies and oversight agencies.
• Acts as a liaison between the community and patient needs; coordinates treatment goals with community agencies, primary care physicians, court system, and managed-care entities.
• May provide supervision to UnityPlace clinicians or supervise counseling or social work students.
• Participates in in-service and competency training of students or staff.
