Job summary

Puget Sound Home Health & Hospice is hiring a Home Health Community Liaison for King County. The role involves 80–90% field travel to hospitals, SNFs, ALFs, clinics, and physician offices, with little time in an office.

You will build referral networks, plan outreach events, and represent the agency in transitional care initiatives across health systems. The position emphasizes relationship-building, meeting referral targets, and serving as a responsive resource for patients and partners.

Qualifications

• Strong communication and presentation skills are essential.
• Tenacity, empathy, emotional intelligence and a solutions-oriented mindset are required.
• Licensed and insured driver with reliable transportation (daily travel is required).
• An associate or bachelor’s degree in Marketing, Business, or health-related science is preferred, but not required.

Responsibilities

• Achieve or surpass referral and admission targets.
• Spend 80–90% of time in the field visiting hospitals, SNFs, ALFs, clinics, and physician offices.
• Build and nurture referral relationships through face-to-face interactions.
• Plan and deliver outreach events such as lunch-and-learn and senior events.
• Represent the agency in transitional care initiatives and strategic relationships across health systems.
• Identify and cultivate prospective patients and referral sources through community outreach.
