The Pennant Group, Inc. in Washington is seeking a Home Health and Hospice Community Liaison to connect referral sources, patients, and our clinical team, ensuring seamless transitions and exceptional care.

This strong sales role requires 80-90% time in the field, with prior medical sales experience (home health or hospice) and an active network. You’ll travel to build partnerships, educate providers, and coordinate admissions while enjoying flexible scheduling and comprehensive benefits.

Qualifications

• An associate or bachelor’s degree in Marketing, Business, or health-related science is preferred.
• Exceptional communication, negotiation, and relationship-building skills.
• Knowledge of Medicare guidelines is a plus, but not required.
• Licensed and insured driver with reliable transportation; daily travel is required.

Responsibilities

• Identify and contact potential clients and referral sources.
• Meet face-to-face with hospitals, physicians, and organizations to expand access to care.
• Educate referral sources on services to ensure timely, appropriate care.
• Coordinate admissions for patients transitioning to hospice.
• Travel to represent the agency in health-system activities and strategic relationships.
• Respond to patient and referral source requests and concerns.
