Job summary

Qualifications

• MSW required.
• Minimum 1 year of clinical social work experience in a health care setting.
• Credentials across DC/MD/VA required within 6 months of hire.

Responsibilities

• Collaborates with health care team to coordinate care and access to cost-effective services.
• Conducts psychosocial assessments and documents findings to guide care.
• Develops and monitors plan of care to improve functioning and quality of life.
• Facilitates access to local, state, and federal programs and resources.
• Maintains documentation per standards and NASW ethics.
• Schedules family meetings and provides counseling and education to members and caregivers.
