Kaiser Permanente in Washington, DC seeks a clinical social worker to coordinate care for a defined member population, collaborating with the health care team to ensure cost‑effective services that align with the plan of care and support the member’s independence and quality of life.

The role involves psychosocial assessment, care planning with the nurse case manager, linking to community resources, assisting with eligibility for public programs, documenting activities, and providing counseling

Formación

• Minimum 1 year of clinical social work experience in a health care setting.
• MSW degree required.
• Experience with Windows-based software is a plus.

Responsabilidades

• Identify and address psychosocial needs; provide financial counseling/referral and guide access to community resources.
• Coordinate with nurse case manager and health care team to develop and monitor care plans.
• Advocate for appropriate resources to meet the plan of care while ensuring cost effectiveness and timely services.
• Document all case management activities in compliance with organizational standards.
• Assist with Medicaid, Medicare, and Disability program applications and refer to community agencies when needed.
• Provide supportive counseling to members, families and caregivers and discuss end-of-life issues and advance directives.
