Job summary

PACS Group, Inc. is seeking a Speech-Language Pathologist to evaluate and treat patients in a post-acute care setting in Waverly Township, Michigan.

You will lead treatment plans, document in medical records, and collaborate with families and the health team to support discharge planning. You must hold a master’s or doctoral degree in SLP, be licensed in the state, and meet state practice act requirements, including supervised practicum and national exam.

Qualifications

• Master’s or doctoral degree in SLP.
• Licensed as Speech-Language Pathologist in the state.
• 350 clock hours of supervised practicum.
• 9 months of supervised SLP services.
• National examination in SLP approved by the HHS Secretary.
• Proficient in Casamba and Point Click Care.

Responsibilities

• Evaluate patients within 24 hours of referral.
• Develop and obtain approval for treatment plans.
• Treat patients per physician treatment plan.
• Supervise CFY/Speech Clinical Fellows when applicable.
• Record evaluations and daily/weekly progress notes and discharge documentation.
• Instruct families or nursing staff in maintenance and caregiver training.
