Job summary

PACE Southeast Michigan seeks an experienced Lead Social Worker (LMSW) to independently assess needs, coordinate services, and oversee social work practice across centers. You will lead the IDT, supervise staff, ensure documentation quality, and support care planning for participants and caregivers.

The role requires MSW, Michigan LMSW licensure, and at least three years of clinical social work experience, with supervision/leadership demonstrated.

Qualifications

• Master’s in Social Work from an accredited program.
• Licensed LMSW in Michigan (active).
• Minimum of three years as a clinical social worker, geriatrics preferred.
• Experience supervising/leading clinical teams required.
• Strong CBT/DBT knowledge and counseling skills.
• Proficient with EHR and essential software tools.

Responsibilities

• Lead daily clinical management and supervise social workers.
• Participate in IDT and coordinate participant care across centers.
• Ensure compliance with regulatory standards and documentation quality.
• Develop and monitor care plans; oversee LOC determinations and discharges.
• Provide coaching, training, and professional development for staff.
• Participate in quality improvement initiatives and audits.
